New Student Registration and Transfers
Pre-K, Kindergarten, and Grades 1 - 5
Students must be 4 years old by September 30, 2009 to enroll in Pre-K.
Students must be 5 years old by Sept. 30, 2009 to enroll in Kindergarten.
                                                                        Registration Information
The following items are needed to register your children in our school:
1. Original Social Security Card
2. Original Birth Certificate
3. Immunization records
4. Drop slip with current grades from last school attended
5. Parent/Guardian Driver's license with current address.
6. Three (3) current proofs of residency (lease, sale, utility bills, etc.) - The guardian and student must live in the
    Denham Springs Elementary school district.  You may go to www.lpsb.org  or CLICK HERE to verify your school district.
7. Custody papers, if applicable.
8. Pre-K registrants must provide proof of income (tax returns & 2 pay stubs)

  * Cartilla de vacunacion

Padres o persona con la custodia.
*Licencia de manejo o crdencial de identificacion con la direccion actual
*3 documentos como prueba domiciliaria
    Recibo de agua
    Recibo de luz
    Recibo de gas
    Recibo de basura
    Recibo de telefono
*Papeles indicando la custodia del menor (si es el caso)
*Si viene de otra escuela/se necesitan las calificaciones
*Documento que indique la salida de la escuela anterior

En caso de no tener los documentos como prueba de domicilio se
requiere informacion firmada por un notario.
